Our approach to performing root cause failure analysis is to use a structured road map (Fishbone diagrams) linked to evidence tables to identify the many potential items which could have contributed to the failure, the evidence needed to confirm these items existed and the actual evidence found to support the inclusion of the specific item as a contributor. The evidence tables also identify any additional analysis or testing needed to support the conclusion that a particular item was present and was either the proximate or root cause.

The deliverable from our root cause failure analysis is a structured report easily understood by the non specialist, complete with Corrective Action Tables which tabulate the actions required to ensure a repeat failure is not experienced and identify those responsible for ensuing the action is completed.
